Background:Aminoglycoside antibiotics have an advantage in their use for the treatment ofClostridium difficilespp. and amoebiasis. The aim of this study was to compare the antimicrobial efficacy of amoxicillin (AOM) in a single dose of Augmentin compared to gentamicin and vancomycin (VA) in patients with moderate to severeC. difficileinfection.

Methods:A total of 875 patients with moderate to severeinfection in the United States (US) were treated with either AOM (n = 533) or gentamicin (n = 533) in a single dose (SD) of amoxicillin for 7 days. Dose of vancomycin (n = 533) and gentamicin was not included in the study. The patients were divided into 3 groups: AOM group, n = 663 and gentamicin group, n = 663 respectively. The microbiological eradication rate (measured by culture) was 100% (n = 663). The efficacy of amoxicillin was compared with that of gentamicin. The rate of adverse events was evaluated by a visual analog scale (VAS).

Results:The rate of adverse events in the gentamicin group was significantly higher than that of the AOM group. The rate of VAS for the AOM group was 3.4 points (95% confidence interval 2.3 to 5.7). The rate of VAS for the AOM group was greater than that of the gentamicin group (P = 0.04). There were no differences between the groups in terms of the rate of adverse events.
